Within the scope necessary for care, we may collect your name, date of birth, address, telephone number, email address, insurance information, medical history and medication history. For self-pay services, we may also handle information necessary for payment.

Please provide the information needed for safe, appropriate care. You may tell us if a question is difficult to answer; however, we may be unable to accept responsibility for disadvantages in care caused by missing information.

Information is used for medical care, billing, insurance claims, patient safety and quality improvement. Research use requires individual explanation and consent or an ethics-reviewed opt-out process, and information is handled in a form that does not identify individuals.
